Exosomes in skincare: what the word covers, and what a label may say

One word, two very different materials

“Exosome” has become a shelf word. It appears on serums at every price, and it is doing double duty: the same term is used for something derived from human cells and for something derived from plants. European law treats those two as opposites. One of them cannot legally be in a cosmetic sold in the EU or the UK at all.

That is not a detail of interpretation. It is a line in an annex, and knowing where it runs tells you a great deal about what is actually in the bottle in front of you.

What an exosome is

Cells release small membrane-bound vesicles into the space around them. Those vesicles carry lipids, proteins and RNA, and they are one of the ways cells pass material to one another. Exosomes are one class of these vesicles, defined by size and by how they are formed inside the cell.

Plants do this too. A plant cell releases vesicles built the same way — a phospholipid membrane around a cargo — and those can be isolated from fruit, leaf or callus culture. That is the material behind a plant exosome preparation: a lipid-and-extract fraction obtained from plant tissue.

The word describes the structure. It says nothing on its own about the source, and the source is what the law cares about.

Where European law draws the line

Human-derived material is prohibited outright

Cosmetics in the EU are governed by Regulation (EC) No 1223/2009. Annex II of that regulation is the list of substances prohibited in cosmetic products, and entry 416 is “cells, tissues or products of human origin”. There is no concentration limit attached and no exception for a purified fraction. Anything sourced from human cells, human tissue or the conditioned media they were grown in is outside what a cosmetic may contain. The United Kingdom retained the same entry after leaving the EU, so the position is identical in both markets.

This is why a serum advertising human stem-cell exosomes may be sold openly in some countries and not here. It is not that European regulators have not caught up. It is that the ingredient is on a prohibition list.

What is allowed instead

Two things sit on the permitted side of that line, and they are often confused with each other.

The first is plant-derived material. A vesicle fraction isolated from apple, grape or centella is not of human origin, so entry 416 does not touch it. It is assessed like any other botanical ingredient: safety assessment, INCI declaration, and the ordinary rules on claims.

The second is synthetic material. An ingredient manufactured to replicate a molecule that also occurs in humans is not itself of human origin, and is permitted provided it is not otherwise restricted and passes safety assessment. A synthesised peptide is the everyday example: hexapeptide-11 is made, not harvested.

Reading the ingredient list to find out which one you have

Under Article 19 of the same regulation, the ingredient list on a cosmetic is written in INCI nomenclature — the standardised names, not marketing names. “Exosome” is not an INCI name. It cannot be, because INCI names a substance by what it is and where it came from, and there is no INCI entry that simply says “exosome”.

So a plant exosome preparation appears on the label as its parts. On ours it reads Pyrus Malus Fruit Extract and Phospholipids: the apple fraction and the membrane lipids it comes with. Grape versions declare Vitis Vinifera; centella versions declare Centella Asiatica Leaf Extract. The front of the pack may say exosome; the back has to say what the material is.

This gives you a test that takes ten seconds. Turn the bottle over and find the botanical name. If the front says exosome and the INCI list contains a plant, you are holding a plant preparation, which is the only kind legally on sale here. If the front says exosome and you cannot find any corresponding entry in the INCI list at all, the claim is not supported by the ingredient declaration — and the ingredient declaration is the part that is legally required to be accurate.

What a cosmetic is allowed to claim

Article 20 of Regulation 1223/2009 forbids text, names, images or marks that attribute to a product characteristics or functions it does not have. Regulation (EU) No 655/2013 then sets six common criteria that every cosmetic claim has to meet: legal compliance, truthfulness, evidential support, honesty, fairness, and informed decision-making.

The practical consequence is a boundary that most exosome marketing crosses. A cosmetic acts on the appearance and condition of the skin surface. It may say that skin looks smoother, that the barrier feels stronger, that fine lines appear softened. It may not claim to regenerate tissue, to signal to cells, to repair damage or to restore anything — those describe a physiological action, and a product making them is being presented as a medicine rather than a cosmetic.

So when a serum promises cellular communication or skin regeneration, the claim is not a bolder version of a permitted one. It is a different category of claim, and it is outside what the product is licensed to be.

What this leaves you with

A plant exosome preparation is a real ingredient with a real membrane structure, sold under rules that require the source to be declared and the claims to stay on the surface of the skin. That is a narrower promise than the marketing around the word suggests, and a more honest one.

Four questions settle most bottles:

  1. Does the INCI list name a plant? If not, the word on the front is unsupported.
  2. Where in the list does it sit? Ingredients are declared in descending order of weight down to 1%, so position tells you roughly how much is there.
  3. What is the rest of the formula doing? Ours pairs the apple fraction with niacinamide, lactic acid, hyaluronic acid and centella — ingredients with their own well-described behaviour on the skin surface.
  4. Does the claim stay on the surface? Anything about regenerating, repairing or signalling is outside what a cosmetic may say.
